According to Dr. Chandana Halaharvi, double board-certified foot and ankle surgeon and owner of Thrive Foot and Ankle, a wide shoe is typically built on the same standard last as a regular shoe, except with a wider midfoot and heel. In these cases, the toe box itself is still tapered. 'A genuinely wide-toe-box shoe is built on a foot-shaped last, meaning that the widest part of the shoe is at the toes, where the foot is truly the widest,' says Halaharvi.
When the toe box is tapered, it can gradually push the big toe medially over years of wear and compress the metatarsal heads, which may contribute to bunions, neuromas, hammertoes, and painful calluses, says Halaharvi. Even for people who don't think of themselves as having wide feet, adequate toe space plays an important role in overall foot function. 'The forefoot naturally splays with weight bearing,' says Dr. Anne Sharkey, board-certified podiatrist and owner of Solely Podiatry. 'This widening is important to aid in balance, shock absorption and forward propulsion. Because of the role in stability, it's important for everyone, regardless of foot shape, to wear shoes with enough room for your toes to splay.'
