Collapse caused severe injuries

According to the Ministry of Labour, Immigration, Training and Skills Development, the worker was operating a forklift to move pallets when a steel storage rack gave way. The collapse caused the worker to fall from the forklift and be struck by falling materials. The worker sustained serious injuries and was transported to hospital for treatment.

An investigation by the ministry found that the rack was not properly anchored to the floor, and the company failed to ensure the racking system was maintained in a condition that would not endanger workers. The rack had been modified without proper engineering approval, reducing its load capacity.

Fine and penalty details

Ideal Supply Company Limited pleaded guilty to one violation of the Occupational Health and Safety Act for failing to ensure the racking system was adequately maintained. The company was fined $80,000 by the Ontario Court of Justice in Kitchener on July 9, 2026. A 25% victim fine surcharge was also imposed, bringing the total payable to $100,000.

“This incident was entirely preventable,” said a spokesperson for the ministry. “Employers must ensure that all racking systems are properly installed, maintained, and inspected to prevent such tragedies.”

Company background

Ideal Supply is a distributor of electrical, plumbing, and industrial products with multiple locations across Ontario. The Listowel distribution centre employs approximately 50 workers. The company has a history of compliance issues, including a previous fine of $55,000 in 2023 for a similar racking violation at its Cambridge facility.

In a statement, Ideal Supply said it “deeply regrets the incident” and has since implemented new safety protocols, including mandatory weekly rack inspections and additional training for forklift operators. The company also said it has hired an external engineering firm to review all racking systems at its facilities.

Broader implications

The case highlights ongoing concerns about warehouse safety in Ontario. According to the Workplace Safety and Insurance Board, there were 1,234 lost-time injuries in the warehousing sector in 2025, a 12% increase from the previous year. The Ministry of Labour has made warehouse safety a priority, conducting over 2,000 inspections in 2025 alone.

“We hope this fine sends a clear message to all employers that worker safety cannot be compromised,” said the ministry spokesperson. “We will continue to hold companies accountable for violations.”
