Windsor Regional Hospital (WRH) has announced the expansion of its innovative Hospital to Home program to two additional healthcare facilities: Hôtel-Dieu Grace Healthcare (HDGH) and Erie Shores HealthCare. This initiative aims to improve patient transitions from hospital to home, reducing readmissions and enhancing recovery outcomes.
Program Expansion Details
The Hospital to Home program, initially launched at WRH, provides patients with follow-up care and support after discharge. By expanding to HDGH and Erie Shores, more patients in the region will benefit from coordinated care plans, home visits, and access to community resources. The program focuses on high-risk patients, including those with chronic conditions or complex medical needs.
Benefits for Patients
Patients enrolled in the program receive personalized care from a team of nurses, social workers, and therapists. This support helps them manage medications, schedule follow-up appointments, and connect with community services. Early results from WRH show a significant decrease in 30-day readmission rates among participants.
Collaboration Across Facilities
The expansion is a collaborative effort between WRH, HDGH, and Erie Shores HealthCare. Staff from all three organizations will work together to ensure seamless transitions for patients. Funding for the expansion comes from the Ontario Ministry of Health as part of a broader strategy to improve home care and reduce hospital strain.
